
Your oral microbiome is the second-largest microbial community in your body and sits at the very top of your digestive tract. When it becomes inflamed and imbalanced, the bacteria and their byproducts travel downstream into your gut and throughout your body, driving inflammation that can stall gut healing. For years, I would have looked only at the gut. Then I started paying attention to something that sits directly upstream of it, a place almost no one thinks to check when digestion is the problem: Your mouth.
Your mouth is not separate from your gut. It is the beginning of the same continuous tube. And the community of bacteria living in your mouth, your oral microbiome, has more influence on your gut health, your immune system, and your whole-body inflammation than most women have ever been told. This article walks through exactly how that connection works and what to do about it.
The oral microbiome is the community of roughly 700 bacterial species living in your mouth, making it the second-largest microbiome in your body after the gut.
You swallow billions of these bacteria every single day. In a healthy mouth, this community stays in balance, with beneficial species keeping the more harmful ones in check. The mouth is not one uniform environment either. The surface of your teeth, the pockets around your gums, your tongue, and your cheeks each host their own distinct microbial neighborhoods.
When that balance tips, a state called oral dysbiosis occurs, in which more inflammatory and pathogenic species take over. Since your mouth is the doorway to your digestive tract, what grows there does not stay there.

This is the connection that reframes so much of gut healing. We tend to picture the gut as a sealed system we can fix in isolation. In reality, it starts at your lips. Every time you chew your food and swallow, you send that day’s oral bacteria straight down into the terrain you are working so hard to restore.
Researchers have increasingly found that oral bacteria can survive the journey through stomach acid and take up residence in the gut, where their presence is associated with disruption of the normal gut community. If your oral microbiome is inflamed and out of balance, you are seeding your gut with the wrong species around the clock, which is one reason a gut protocol can stall even when everything else is being done right.
Most women working on their health have heard of leaky gut, where the lining of the intestine becomes more permeable than it should be and lets particles slip through that trigger immune activation. The gums can do the same thing.
When the tissue around your teeth becomes chronically inflamed, that barrier turns permeable too. Some practitioners call this leaky gums. The bacteria and their inflammatory byproducts, including a bacterial toxin called endotoxin, can enter the bloodstream directly through the gum tissue. It is the same principle as leaky gut, just higher up the same system.

Once oral bacteria and their byproducts enter the bloodstream, they can travel to distant organs, where research has linked them to inflammation in the brain, arteries, and joints. This is where the oral microbiome stops being a dental issue and becomes a whole-body one.

The Brain
In a widely cited 2019 study published in Science Advances, Dominy and colleagues identified Porphyromonas gingivalis, a keystone bacterium in gum disease, in the brains of Alzheimer’s patients. In animal models, oral infection with this bacterium led to brain colonization and increased production of amyloid beta, a component of the plaques associated with Alzheimer’s.
The Heart
Oral pathogens and the inflammation they generate have long been associated with cardiovascular disease, and periodontal bacteria have been detected within arterial plaque.
The Joints
This one matters enormously for the women I work with, because autoimmune conditions like Hashimoto’s and rheumatoid arthritis show up so often in this community. There is a well-documented link between gum disease and rheumatoid arthritis, and the mechanism is fascinating.
Porphyromonas gingivalis produces an enzyme that citrullinates proteins, meaning it chemically alters them. Your immune system builds antibodies against these altered bacterial proteins, but because they closely resemble your own citrullinated proteins, those antibodies can cross-react and start attacking your own tissue. This is a process called molecular mimicry, and researchers at Johns Hopkins and elsewhere have detailed it as a plausible bridge between periodontal disease and rheumatoid arthritis.
One meta-analysis found that people with periodontitis lasting more than five years had a significantly higher risk of developing rheumatoid arthritis than those without it. In clinical terms, this is why getting periodontal disease under control has been observed to reduce inflammatory markers. The mouth is not just a reflection of immune dysregulation. In some people, it is helping drive it.
One of my clients had done everything. A GI Map, a food sensitivity panel, two rounds of elimination diets, and a cabinet of quality supplements. She was still bloated by dinner and too depleted to show up the way she wanted to for her family.
