Just because fatigue is widespread does not mean it is acceptable, nor does it mean you have to live with it. In most cases, persistent exhaustion has an underlying cause, and the right labs for fatigue can help you find it.

Chronic fatigue is persistent tiredness that lasts for weeks or months and is not relieved by rest or sleep. It interrupts your daily life, your responsibilities, and the things that matter most to you.
Many cases of chronic fatigue negatively impact physical, mental, and emotional performance. You might sleep eight full hours and still wake up feeling like you never slept. You might rely on caffeine just to function, or crash every afternoon no matter what you do.
As with any symptom, the goal is never to chase the tiredness itself (which is just a band-aid). The goal is to identify the root cause. Most often, chronic fatigue stems from more than one cause at the same time, including:

That gap exists because standard reference ranges are built to flag disease, not to define optimal function. A result can sit at the very bottom of the “normal” range and still leave you exhausted.

Chronic Inflammation and Immune Dysregulation
Chronic inflammation is slow, long-lasting inflammation that impairs many other functions. Persistent immune activation can lead to what researchers call cytokine-mediated fatigue, where inflammatory signaling molecules directly drive the feeling of exhaustion. Autoimmunity, in which a dysregulated immune system attacks the body’s own tissues, can further deplete energy.

Low iron is one of the most common findings in tired women, but iron supplementation can sometimes make fatigue worse rather than better. The reason is that low iron on a lab is a clue, not a diagnosis, and giving more iron without understanding why it is low can backfire.
This is one of the most important and most misunderstood pieces of the entire fatigue conversation, so I want to slow down here.
When most people see a low iron or low ferritin result, the instinct is simple: take iron. But here is what often gets missed.
First, iron is a pro-oxidant. When you flood the body with supplemental iron that it cannot properly use, it can increase oxidative stress, the very thing that damages your mitochondria and drains your energy.
Second, iron feeds bugs. Many bacteria, including gut pathogens, require iron to grow and thrive. If you have an underlying gut infection (more on this below) and you start aggressively supplementing iron, you can inadvertently feed the very organisms making you sick, worsening inflammation and fatigue.
Third, and most importantly, much “low iron” in chronically inflamed women is not a true deficiency of iron in the body at all. It is a problem of iron being locked away where the body cannot use it. Pouring more iron in does not solve a locked door. This brings us to iron recycling and inflammation.
Your body is remarkably efficient with iron. Rather than relying only on what you eat, it recycles the iron from old red blood cells every single day, using specialized immune cells (macrophages) to break them down and return the iron for reuse.
This recycling is governed by a master regulator hormone called hepcidin. Here is the key functional insight: when the body is inflamed, hepcidin rises. High hepcidin signals the body to reduce iron absorption from the gut and lock iron away in storage, keeping it out of circulation.
The body does this on purpose. Sequestering iron during inflammation is partly a protective strategy to starve pathogens. But the downstream effect is often called functional iron deficiency, or anemia of inflammation, which means you have iron in the body, but it is trapped and unavailable, so you still experience the symptoms of low iron or anemia.
In this scenario, giving more iron does not fix the problem because the problem is not a lack of iron. The problem is the inflammation driving hepcidin up. The real work is to find and address the source of inflammation, and the iron and anemia picture often improves on its own.
This is also why ferritin, the marker most people use to assess iron stores, can be so misleading. Ferritin is an acute-phase reactant, which means it rises in response to inflammation. A woman with significant inflammation may show a “normal” or even high ferritin level while being functionally iron-deficient. Reading ferritin without also looking at inflammatory markers is one of the most common ways iron status gets misread.
